To ensure quality service please read the following office policies and information:
Office Hours:
Please feel free to call my office Monday through Friday between the
hours of 9 PM to 5 PM. If I am not available I will return your call within 24 hours.
Cancellation Policy:
There is a 24 hour policy for cancellations or rescheduling. Patients will be
responsible for missed appointment fees.
Fee Schedule
First Visit: $75.00. Thereafter: $60/visit

Payment:
Payment is expected at the time of service. I accept cash or check, No credit or debit cards at this time. The first visit is 75 dollars and
each subsequent visit will be 60 dollars.
Insurance:
I will be glad to make out a receipt for you to submit to your insurance provider. I belong to the network of National Health
Administrators.
Inquires:
If at any time you have any questions or concerns about your treatments, please feel free to bring them to my attention. I am happy to accommodate you in
anyway that I can.
